Document Checklist: "Schedule A" Green Card Application
Resume
Please provide an updated Resume including the following information for each nursing job you have held during the previous five years:
Employer Name
Employer Address
Job Title
Employment Start Date and End Date
Number of Hours Worked per Week
Note: Although this information may be a bit tedious to prepare, it is required to complete your immigration application forms.
Nursing Diplomas & Transcripts
Please provide copies of your nursing diplomas/degrees and associated transcripts from all nursing schools you have attended.
NCLEX
Please provide a copy of your NCLEX Candidate Report (see example) or other official documentation confirming you have passed the NCLEX examination. If you provide a letter from a State board of nursing, the letter should specifically indicate that you passed NCLEX.
Nursing License
Please provide a copy of your Nursing License issued by the U.S. state where you work. (If you do not have this document yet, please confirm.)
VisaScreen Certificate
Please provide a copy of your VisaScreen Certificate issued by CGFNS. (If you do not have this document yet, please confirm.)
Passport Pages
Please provide a copy of your passport identity page and any pages with a U.S. visa sticker and/or entry stamp.
Note: You do not need to provide copies of blank pages.
I-94 Card
Please provide a copy of the front and back of your I-94 Card issued by U.S. Customs and Border Protection.
Note: Only Canadian nurses working in the U.S. in TN work visa status will be issued a paper I-94 Card. I-94 records for all other nurses are now stored electronically online.
Nurses outside the US will not have an I-94 record.
Family Documents
If you have a spouse and/or children that live with you in the US currently, or will be moving to the US with you, please provide copies of the following documentation:
Copy of your marriage certificate
Copies of the passport identity pages for your spouse and/or children and any pages with a U.S. visa sticker and/or entry stamp.
Copies of the I-94 Cards for your spouse and/or children (if applicable)
Note: If you currently live in Canada and are applying for a Commuter Green Card, you do not need to provide copies of these documents for your family members.
