Document Checklist: "Schedule A" Green Card Application
Please provide an updated Resume including the following information for each nursing job you have held during the previous five years:
- Employer Name
- Employer Address
- Job Title
- Employment Start Date and End Date
- Number of Hours Worked per Week
Note: Although this information may be a bit tedious to prepare, it is required to complete your immigration application forms.
Please provide copies of your nursing diplomas and/or degrees from all nursing schools you have attended.
Please provide a copy of your Nursing License issued by the U.S. state where you work.
Please provide a copy of your VisaScreen Certificate issued by CGFNS.
Please provide a copy of your passport identity page and any pages with a U.S. visa sticker and/or entry stamp.
Note: You do not need to provide copies of blank pages.
Please provide a copy of the front and back of your I-94 Card issued by U.S. Customs and Border Protection.
Note: Only Canadian nurses working in the U.S. in TN work visa status will be issued a paper I-94 Card. I-94 records for all other nurses are now stored electronically online.
Please provide a copy of your birth certificate.
Note: If you were born in Canada, you must provide a copy of a full-size "long form" birth certificate. U.S. Citizenship and Immigration Services (USCIS) does not accept the wallet-sized, "short-form" birth certificate as official birth evidence in connection with a green card application. If you do not have a "long form" Canadian birth certificate, you must order one from your Province government. "Long form" birth certificates for individuals born in Ontario can be ordered here.
If you are currently living in the U.S. and you have a spouse and/or children, please provide copies of the following documentation:
- Copies of the passport identity pages for your spouse and/or children and any pages with a U.S. visa sticker and/or entry stamp.
- Copies of the I-94 Cards for your spouse and/or children
- Copy of your marriage certificate
- Copies of your children's birth certificates
Note: If you currently live in Canada and are applying for a Commuter Green Card, you do not need to provide copies of these documents for your family members.
